Appointments

Patients are seen by appointment only. Please call (303) 790-1800 to schedule an appointment. Appointments are normally made by physician referral, however, self-referrals are accepted in cases where the patient is seeking a second opinion or has known neurosurgical problems.

 What to bring to your appointment

  • Insurance card
  • MRI or CT (the actual films or DVD)
  • List of your current medications
  • Referring doctor's address, phone, and fax number
  • Referral, if required by your insurance

We accept most commercial insurances, Medicare, and Colorado and Federal workers’ compensation. You will be required to have your insurance card and co-pay available at the time of your visit. We will assist you with the billing of your primary insurance. Letters of protection are not accepted.

Authorizations

It is your responsibility to ensure whether a service is covered on your insurance plan. You may need to contact your insurance company to determine this before making an appointment.

Confidentiality

We keep all patients' medical records completely confidential. The records can only be released with your written authorization. We cannot discuss your medical condition and care with anyone except those who are authorized.
